The Department of Medical Services and Hospital provide initial care for all emergencies occurring at the MSU campus. It is also available to all students for consultation on medical/health and dental problems. In general, this medical care does not involve additional cost to the students except where resources outside the Health Services must be utilized.
Rules and Regulations/Policies Enrollment
All new students are required to undergo thorough Physical Examination and are thus issued their respective Entrance Health Examination Forms (Green form for male students; Orange from for female students). After filling up the form, the student then submits himself/herself for blood pressure checkup, pulse rate and visual examination. From the Dentist the student goes to the Physician on Duty for medical examination after which he proceeds finally to the Record Section located at the Hospital Administration Building for the approval of his/her Preliminary Registration Form.
Consultation (Medical & Dental)
For a student to be promptly attended to or properly acknowledged, he/she is advised to always bring along his/her updated Card to be shown to the Records Clerk at the Record’s Section, of the Hospital Administration Building who then will release to said student his/her medical health Form. He/She then goes either to the Treatment Section for medical consultation or the Dental Section for Dental Consultation. Consultation Hours shall be followed strictly; Morning – 9:00 A.M. to 11:00 A.M. ; Afternoon – 1:30 P.M. to 5:00 P.M. Mondays to Fridays only. Only emergency cases will be entertained after office hours or during Saturdays, Sundays and holidays.
A student maybe confined upon recommendation of the Physician On-Duty. Visiting time: Morning – 9:00 A.M. to 10:00 A.M; Afternoon, 4:00 P.M. to 6:00 P.M. Only 1 – 2 watchers are allowed. A female patient is allowed female watchers only. Tapes and radios are not allowed inside to avoid disturbing other patients. Vandalism and absconding is prohibited. Discharge Time during Office hours only. Office hours are from 8:00 A.M. to 12:00 P.M.; 1:00 PM to 5:00 P.M..
Issuance of Medical Certificates
A. Sick Leave of Absences
A medical certificate will be issued only to students who seek consultation/confinement in the Infirmary. Any medical certificate secured outside or issued by either a private or a government physician outside or the Infirmary physicians will have to be noted by the Medical Director after thorough evaluation and assessment of said medical certificate. Sick leaves up to (5) days will be issued by the Physician On Duty while those exceeding five (5) days will be issued by the Medical Director.
B. Detail at the DAST Office for ROTC/Adopted PE
Depending upon the extent of the incapacity or ailment of any student concerned, anybody seeking Medical Certificate for detail at the DAST Office or wishing to take up Adopted PE will be scrutinized and subsequently recommended by the Medical Committee composed of 3 Physicians and 1 Dentist to be submitted to the Medical Director for final approval.
